For treatment. To provide and coordinate your care, including with the labs, pharmacies, imaging facilities and partner clinicians involved in it.

For payment. To bill you and process your payment. We are cash-pay. We do not bill insurance, and no federal healthcare program money is involved in anything we do.

For healthcare operations. Quality review, training, and running the practice.

As required by law. Including public health reporting and lawful requests from authorities.

Your rights

  • To inspect and get a copy of your record, usually within 30 days.
  • To ask us to correct something you believe is wrong.
  • To ask for a list of certain disclosures we have made.
  • To ask us to restrict how we use or share your information.
  • To ask us to contact you a particular way or at a particular address.
  • To a paper copy of this notice, even if you agreed to receive it electronically.
  • To be notified if your information is breached.
